I like to start with scenes. Intense moments or developments. Then sketch out the characters required to produce the personalities that cause such intensity, then shape the values that drive those personalities, and flesh out circumstances that would lead up to the original, intense scene. Not all stories have those scenes, but sometimes, a character or theme is born just from a painting, or the way a woman plays with her hair, or how a mouthful of fresh air tastes like in an isolated mountain.
