First thing I'd suggest is go meet with all of your teachers and discuss what you can do to improve. Sometimes if you force yourself into getting things done, moving forward, getting caught up, it motivates you all on it's own. Go to each and every teacher. They will respect you for tying to get caught up. As for math, go to your math teacher and ask if they have tutoring hours available, etc. Read in your textbook and pay really careful attention in class and ask questions.
